During an FCA, our Occupational Therapists will observe your abilities, ask questions, and conduct standardised assessments. We consider all aspects of your life, including medical history, responsibilities, and any barriers you may face.

What’s Included in an FCA Report?
An FCA report typically includes:
- Identification of your strengths
- Assessment of limitations that may impact independence
- Recommendations for support hours, equipment, or therapies required in an NDIS plan
- Guidance for your NDIS planner and family on current and future support needs
When Would You Need an FCA?
You may require an FCA when:
- It’s included in your NDIS plan to guide future services and transitions

- You or your family request it for transitioning to the NDIS, or for reviewing or appealing an NDIS plan
